Service Improvement
The Integrated Care Board (ICB) led a system-wide improvement programme to address delays in Emergency Department (ED) care for frail older adults. Working with acute and community partners, Frailty Assessment Units (FAUs) were established within EDs, introducing frailty screening at triage and early access to therapy and discharge planning. Prompted by a patient’s 14-hour ED wait, the initiative improved collaboration across teams. Within six months, admissions for patients aged 80+ fell by 30%, and assessment times improved significantly.

The Trust set out to strengthen its response to deteriorating patients after the unexpected death of an individual with complex comorbidities. Leaders wanted to improve the reliability of early sepsis recognition, ensure timely escalation, and embed learning across teams. A Structured Judgement Review highlighted missed NEWS2 scores, delays in sepsis bundle initiation, and limited senior oversight in the first 12 hours of admission.
In response, the Trust implemented mandatory sepsis huddles, launched targeted ‘recognise and rescue’ training, and upgraded electronic alerts. The patient’s family was engaged throughout, with their feedback shaping internal safety messaging. Within six months, sepsis bundle compliance improved by 23%, and staff reported greater clarity and confidence in escalation pathways.
A Mental health Trust commissioned an independent investigation to strengthen the early recognition and response to physical health deterioration among patients with serious mental illness.
Aiming to address long-standing disparities in physical health outcomes, senior leaders sought to embed a culture of shared responsibility, sustained clinical focus, and learning.
